Join TLHS

Whether you’re a freshman or a senior, you can join the TLHS. There are two ways to do it:

  • Fill in the form below and we’ll send you a Health Behavior Survey and consent form.
  • Complete the survey and consent form on the web. (Coming Soon)

When we receive the completed survey and signed form, you’ll be assigned a numberical study code and that’s it, you’re in.
